Leicester and Man Utd are level on 62 points in fourth and fifth, but Man Utd have a match in hand, at home to West Ham United.
Victory at Old Trafford on Wednesday would mean they would go into their final-day trip to Leicester knowing at least a draw will earn them a return to the UEFA Champions League.
Chelsea are third, a point above Leicester and Man Utd, but cannot relax as they face two tough final fixtures.
They first travel to the home of champions Liverpool before they host a Wolverhampton Wanderers side chasing European football of their own, albeit in the UEFA Europa League.
If Chelsea were to get only a point from those matches, Man Utd win on Wednesday but then lose to Leicester next Sunday, it will be the Blues who slip down to fifth and a UEFA Europa League spot.
